I crediti formativi ECM saranno certificati dal Provider OIC srl secondo la nuova regolamentazione approvata dalla Commissione Nazionale per la Formazione Continua il 13 gennaio 2010. Si ricorda che avranno diritto ai crediti ECM solo coloro che saranno presenti per l’intera durata dell’evento ed avranno compilato l’apposito questionario che verrà consegnato in sede congressuale. Si ricorda inoltre la determinazione assunta dalla Commissione Nazionale per la Formazione Continua del 18 gennaio u.s. alla luce della quale ogni partecipante potrà maturare 1/3 dei crediti formativi ricondotti al triennio di riferimento (150 totali per il triennio 2012-2015) mediante reclutamento diretto da parte dello sponsor e dovrà consegnare al Provider dell’evento una copia dell’invito o una dichiarazione sottoscritta attestante l’invito, con firma autografa e leggibile unitamente ai propri dati anagrafici (codice fiscale, nome, cognome, libero professionista / dipendente / convenzionato, professione, disciplina, nome sponsor). A tal fine è stata predisposta una scheda riepilogativa nell’ambito del questionario ECM. L’attestazione dei crediti formativi ottenuti sarà inviata agli iscritti dopo l’evento a seguito della verifica da parte del Provider del raggiungimento del 75% di risposte esatte. A tutti gli iscritti sarà comunque consegnato il consueto attestato di frequenza. Soltanto i relatori che abbiano svolto una relazione nell'ambito del programma scientifico accreditato per almeno 1/2 ora continuativa potranno richiedere i crediti docenti in ragione di 1 credito per ogni 1/2 ora. Non sono previsti crediti per tutti gli altri relatori e moderatori dell'evento. I crediti formativi sono rivolti a medici chirurghi specialisti in anestesia e rianimazione, cardiologia, cardiochirurgia e a infermieri e tecnici della fisiopatologia cardiocircolatoria e perfusione cardiovascolare.

REGISTRATION FEE:

Registration fee includes VAT 21% and includes also:

Admission to Scientific Sessions, Congress Kit, Echo and Simulation Course (upon request, to be chosen upon registration), Working Lunches, Coffee Points, Welcome dinner April 28th, 2012, Social Dinner April 29th, 2012, Live Cases Book, Certificate of attendance.

IMPORTANT:

Registration will be accepted until April 14, 2012. After that date, registration will be accepeted only on site.Registration is mandatory for partecipation in the Scientific Session.

HOTEL ACCOMMODATION:

Price are per room and include VAT and breakfast.

Confirmation of Reservation

Please note that to confirm the hotel reservation your pre-payment must be received by April 14,2012. Upon receipt of your full payment, O.I.C. Way Srl will send you the details of your reservation including hotel name and full address. In case the balance due is not received by the above indicated deadline, the reservation will not be confirmed.

Hotel reservation will be managed by OIC Way Srl, official Housing Bureau of the Congress that will be at your disposal and will send you a complete proposal following availability.

Full payment for the booked period (including Euro 25,00, VAT included, for the booking fee) is requested in order to ensure that your hotel reservation is confirmed properly. Telephone reservations will be not accepted.

CANCELLATION AND REFUND POLICY:

In case of cancellation of registrations and/or hotel reservations, please apply in writing to the Organizing Secretariat OIC Srl.

COFFEE BREAKS AND LUNCHES:

Coffee breaks and lunches will be offered to all regularly registered participants.
